The picture show an histological image of a plaque removed from the main artery in the neck (the Carotid artery). The different colors and structures indicates different types of tissue (e.g. Collagen rich tissue, Collagen poor tissue). When the plaque was inside the patient, the artery wall was located outside of the colored area, while the blood was flowing in the residual lumen in the center of the picture.
